I think we are allowed to speculate what may have failed, that's all we can do until they are more forthcoming. At this point, at least speaking for myself, I don't believe it was the A.D.D that broke, I think it was something internal.
Regardless of what anyone says past this point (TFL or Toyota), I will find it hard to believe they replaced the entire diff due to a simple actuator failure, and the tech notes imply the actuator was working upon inspection, or at least moving.
I could be wrong, it just seems completely counter-intuitive and unnecessary to replace the entire diff/axle if it was the A.D.D. The A.D.D bolts on simply by 4 bolts, it's not really part of the differential. It's also possible Toyota was in a bit of a panic due to TFLs reach/audience size and they just wanted every part imaginable in the chain of parts that could be affected as forensic evidence.
I'm guessing Toyota will have some follow-up, maybe with TFL doing the delivery for them. It will be interesting to watch.
